Discounted Package includes:

●       Care Management 

○       3 hours of Care Management (MSW) services

○       Services provided:

■       MSW will meet client at the hospital

■       Review discharge plan with client and hospital staff

■       Transport home

■       Call and schedule follow up appointments and transportation

■       Contact Home Health and assist with DME orders

■       Home safety inspection with recommendations and referrals

●       In Home Care

○       8 hours of In home Care 

○       Services provided:

■       Companionship

■       Personal care

■       Light housekeeping

■       Meal preparation

■       Shopping for food and medications

■       Medication reminders and assistance

●       Program Additions

○       Option to add up to 3 more hours of Care Management at discounted rate

○       Option to add up to 5 more 8 hour shifts (total 48 hours) at discounted rate

○       Transportation and companion to one follow up appointment at discounted rate

●       Optional services from Referral partners

○       Life alert for one month at a discounted rate

○       Prepared meals for one week for 2 people or two weeks for 1 person at a discounted rate
